One of the procedures that we do in our office a fair amount is tooth colored braces. It’s called Six Month Smile.  Six Month Smile gives our patients the ability to have cosmetic braces and have a really nice outcome in what we do with that. What do I mean by that is moving teeth at a fairly rapid pace to get the teeth in alignment.

Today I have a pretty significant patient. Her name is Jamie.  Jamie has agreed that we can do this online and post this. What I want to do is to ask Jamie a few questions and then have her answer those.

You can show us the how you were beforehand and how you feel now that this is done and maybe some changes that have taken place as well as our relationship with you in our office.

So, this picture over to the right is what Jaime looked like about a year ago.  This case took longer than six months. It actually took over a year. But we see that Jamie had extra teeth in her mouth. She had baby teeth.

She had teeth that didn’t show up here. Her eye teeth were here. Her bite was way over. And now, this is what Jamie looks like now.

This is the difference that we’ve seen in Jamie. You can see Jamie’s nice smile there. There we go, see?

Jamie, it seems like there’s been a complete change in the way that you were feeling so just tell us a little bit about how you found out about us and what your experiences were like.

Jamie: When I first came into Dr Antolak’s office I had major fear of dentistry. I had some issues as a child with anxiety going to a dentist and was just never able to find a dentist that I was comfortable with. When I came here, I remember one of the associates taking me back to take before pictures and told me to smile as a before picture and I told her I didn’t know how to.

So, now I can’t stop smiling.  I really can’t.

I really like the office because when I come in, everyone knows who I am. It’s not a big chain office. Everyone knows each other. The staff gets along really well.

You can always tell when there’s good stuff and bad stuff going on, or when there’s a bad doctor-patient relationship and all that kind of stuff.  This is definitely the most comfortable office that I’ve ever been in.

Dr Antolak: That’s fantastic and you know, you do smile.  In fact, this is the reason why we had to do a picture with her lips pulled back, because we couldn’t force her to smile like that at the time, right?

We understand that recently you just got engaged?

Jamie: I did.

Dr Antolak:  So now you feel like smiling but what have been some of the reactions from people?

Jamie: One of my coworkers, actually the day that I got my braces off, she couldn’t stop staring at my mouth. She was just amazed because she had seen my before picture, when I had first started working there. And she was like, I just can’t stop staring at your mouth. She’s like, I’m talking to your teeth! I’m like I know, I am too.

Dr Antolak:  That’s fantastic. Thank you so much for that.

Jamie:  Yes, thank you.

Dr Antolak:  And this is the type of thing that can be done. In Jamie we had a really nice change, huge change.

Jamie:  Huge doesn’t even start to talk about it.

Dr Antolak:  The best part about it is the way that you feel about yourself and the fact that you can smile in public now and feel good about that.

We’re going to be doing some cosmetic dentistry on her today at our Shelby Township dental office.  What I would like to do is just explain some of the reasons that you are looking forward to having this done and maybe some of your experiences so far.

Shelby Township Dental Patient:  Well the reason I want to have it done is because I struggled with, not liking my smile my entire life due to having my teeth broken off when I was young.   Then, I had to have crowns done at a very young age and then I had to have them replaced.  None of my teeth have ever matched and it definitely prevents me from smiling.  I mean I do smile but not to the fullest extent. You know, I’m always conscious of people looking at my mouth. And, I’m just ready to change that.  So far, Dr. Antolak has been terrific. The dental office is very enjoyable and I’m looking forward to having it all done.

Shelby Township Dentist:  Good, good, yeah, we’re looking forward to this too. We’re going to have a few challenges with the fact that she did break her teeth off and she’s got a metal post in there.  So we’re going to have to do some things, to give her a nice, very natural appearance to her teeth. As we go through the cosmetic dental procedure I’ll explain what’s going on.

Shelby Township Dentist:  So right now, what I did is I just added some composite, some tooth colored composite material to this tooth here and her eye tooth here, and the purpose of that is just to bring these teeth out so they look a little bit more prominent.  Previously, we had done some bonding on these teeth here to brighten them up and on the back ones, so we can have a nice, uniform color to her teeth. So now what we’re going to do is we’re going to take an impression of these, and we’re going to duplicate this and then, take off these crowns.

Shelby Township Dentist:  So what we have here is, we went ahead and I took her old crowns off. This is where she fell and hit her tooth and this has got what is called a dental post in it. It’s got actually a metal post and core. And so we have to try to achieve the same underlying structure.  This is called a stump shape like these two are and we want to try to achieve that on this tooth. So I’m going to do some special things to bond some composites, block out this color and try to achieve this color closely so that when these all porcelain crowns are made that they all look the same.

Another issue we have here is with her eye teeth. Eye teeth typically are darker. This one is and this one is. So, we also have to take this color and so we can achieve our final result which we’re trying to get something like this as a color. This is actually a bleach shade. So we’ve got to go from this, color and make it look natural along the process. We’ve got to go from this color and make it look natural along the process so I’m going to do some composite. I’m going to bond some material onto this tooth here so that it can, like I say, try to match the adjacent teeth.

Shelby Township Dentist:  We are going to do the dental impression now.  We are putting this material in her mouth. Put it up there, and it’s just going to stay in there for about 4 minutes or so.  Up to this point, she’s been using the gas, and she’s been doing really well. The dental procedure’s going very smoothly.

So what we’re going to do now is, we’re capturing the impression of her teeth. We’re going to take a photo of the color that we have underlying what’s going to be the dental crown and the two porcelain dental veneers. We’re going to then send that to our laboratory, and he’s going to then fabricate the porcelain off of that. So, we’ll see what that looks like, and we’ll make some temporaries out of a light shade and we’ll see, best we can get make it look for short term while the temporaries are in there, while [indiscernible] is processing the, the porcelain.

Shelby Township Dentist:  We went ahead for Colette, and put the temporary on for her, and we are going to be sending this off to laboratory. The color of it is going to be a little bit brighter than we currently have. They look very nice, very natural. right now, she is completely numbed up, so you’re not going to. You’re still numb from before so your lips not going to have the mobility.  Here is a mirror so take a look and see.

Shelby Township Dentist:  So, what I did was I went ahead and I brought these eye teeth out a little bit more from where they were, okay, to give you a full, really nice broad smile too. Like I said, the color’s going to be the color of the teeth next to them are going we’re going to brighten those up a little bit more from where they currently are. But you actually have good mobility with your lip. You know, you can’t feel them, but they still move.

Shelby Township Dentist:   So we went ahead and put those on for you, and go ahead and take a look and see what they look like.

Shelby Township Dentist:   Oh my gosh, you look so nice. So we did some beautiful nice dental veneers and some porcelain dental crowns. We masked some dark teeth that you had from a long time ago. And so it’s going to take some time to get used to being able to smile confidently in in front of the camera too.

Hi, this is Doctor Antolak from thegentledentist.com in Shelby Township, Michigan, and what I’m holding here is a model, a diagnostic model of one of our patients. I do quite a bit of cosmetic dentistry, as you can see from our website, and we’ve actually been able to help a lot of people with their smile and improving it.

What I want to do is to show you on a detail of what we do before we start procedures which are more complex. As we take impressions of the mouth, that’s that kind of that gooey stuff that can go in the mouth, it captures a negative image of that. Then we take that and we pour that up in plaster.

This is a plaster model here, as you can see. These are made out plaster and it’s an exact duplicate of this patient’s mouth and what their teeth look like. What it does is, this is called an articulator. An articulator duplicates what it would be like in the jaw. So this shows the jaw joint here. If we need to move the teeth around so we can diagnose what the person’s problem is, then we can move the jaw in different directions. I can see forward and backward like this. We can move it from side to side. We can open and close it. What this does is allows us to look and see where the teeth are lined up and what potential problems they might have and it gives us an ability to have what’s called a working model. Something like this. We can do some prototype work so that when that patient comes into that room, we are ready to treat them and do their smile or implants, for example, because we have done it already and the details are worked out in the plan before anything is really done.

What’s important about that?

It’s so important to start with the end in mind. You have to start knowing what your result is going to be. So that you know what the next step is. I’ve had patients of mine that come to me and say they had this done and this done, and then they had had something redone because it wasn’t planned out properly. So what we do at our office is, we take plenty of information, you know we want to interview our patients, so that we will know what their desires are.

We don’t assume anything and so after we get done interviewing, we take photos, a lot of high definition photographs, so that we can see what their teeth look like. The position of the lip, the way the lip looks when they’re lips are at rest and relaxed, when they’re smiling, from either side. We take pictures of the jaws from the top or the bottom and what that does is it gives us a really great ability to see as if that patient was actually in the chair. Sometimes, we can even do more information once we have models like this, photographs and x-rays, to plan for the best result possible.

Once again, you have to start with the end in mind and that end is dictated by you, the patient,, who will tell us what you want for yourself, the way you want things to look.

There are a number of techniques that can be used to enhance your smile:

Well, this is called an articulator, and this device right here is called a face bow. And what these are used for is to accurately mount a plaster duplication of someone’s jaw and teeth.

This is required because it gives us a more accurate impression and a more accurate, what we call a mounting, so that the results that we do for our patients are more accurate and precise.  So, if this is something that would need to be done, we would actually put wax on this top of this here. Soft wax that would go in the valve and it goes like this, okay. And then goes right up to the ears and goes in the ear canal. And what we do is we adjust these things here; we adjust this back and forth so, then we create the relationship between, right here is where the ear canal is and right here is where the upper teeth are. Then we take that and we mount, we take it off of here and then we place it on here and then we mount the teeth to there, and so what that does- it creates an accurate 3-dimensional duplication of our patient, so with that I can take that information and then transfer to our laboratory. And then when they make it, they can have very accurate representation of ways the jaw moves back and forth and how it moves up and down.  So it goes back and forth like that and then we can do a mounting and that type of thing.

This is the type of work here that we do. We communicate with our laboratory in Macomb County, Michigan; actually, it’s called Macomb Dental Ceramics. So it’s just located a few miles from our office so we can do this accurate information, and get a very good result using our digital cameras for pictures, and also for this type of accuracy in those cases for different cosmetic procedures.

Dental Whitening is Discussed by Dentist in Macomb County, Michigan

Hi. This is Dr. Robert Antolak at thegentledentist.com in Shelby Township, Michigan, and what I’d like to show you is the effects that we can have from doing whitening on people’s teeth. You know, whitening is one of the most recommended and one of the most requested procedures that we do to help with cosmetic dentistry.

Many times we’ll do an arch of teeth. For example, if we do the top teeth making it nice and white the bottom teeth sometimes are a bit dingy. So what I’d like to do is to talk, to show you a way that we whiten the teeth that is like nothing else. That’s called KOR White. This is just an example of some of the pictures that KOR White can do. So, for example, these are the same color tabs here. This is before, and then after the things that can be done. The difference between  KOR Whitening, and some of the other systems, is that it’s designed, and formulated for those people who are sensitive to their teeth, as they start weighting their teeth, their teeth can sense it.

The reason why that is, is because of the way that it’s packaged. KOR whitening is a product that is for transportation, they actually need to have it in refrigerated form, because that is a preservative, and the reason why it needs to be preserved with refrigeration is that it’s at a neutral pH.

Other products that are used, use an acidic pH to be preserved, and so, in other words, an acidic pH, wen we want to bond teeth, we use an acid that etches the teeth, which can make them sensitive and this product is at a neutral pH, so it is not as harmful to the teeth, as far as making them sensitive afterwards. And another reason why it works so well is because just the formulation and how even is is, these are just some examples of what to expect. With this sheet here and then this is after the whitening. One other thing with KOR whitening is that it does not require a light.

Many times the light that is used to intensify the bleaching agent will actually create the heat, will actually create sensitivity to the teeth. So, this works really well to help reduce that and yet still get a very nice result. So if you would like the alternative and if you have really pretty dark teeth and you’d like to see a change in that, then you can give us a call and we’d be more than happy to talk to you about that as an option.

The one difference is, the KOR whitening costs about twice as much as the regular whitening, and the reason why is the cost of the materials themselves. We buy it and it is much more expensive than it would be compared to an over the counter or, you know, whitening agent that we use, which is effective, but maybe not as effective as a KOR whitening product is.

So, if you would like to get an assessment with your color of your teeth- by the way, the color of the teeth that will whiten the most are teeth that are, have a yellowish color to them. And for myself, I did that to myself. So I’ll show you here. I haven’t retouched my top teeth in probably over a year. But this is my top and these are my bottom teeth. See, and I whitened my top, and I didn’t whiten my bottom. See that? And so, people ask, why did you do that? And that way I can show our patients the color of the teeth and what the effect that whitening can have.

But basically, if someone comes in, this is not Veronica’s mouth, but if it were, this is what her teeth would look like.

So, basically, we’ve got teeth here, missing teeth, and the Snap on Smile is made. It’s all one piece. It’s very, very durable; you can actually use it where it snaps over the teeth. So we can change and lengthen teeth. We can fill in the gaps on some of the teeth and we do it with just one component. It’s not a permanent solution, but it’s one that does last a while, and we’ve been able to have some very favorable results with the Snap on Smile.

One very unique indication we’ve had is with Veronica and I asked if we could just do an interview about some of the things that have taken place for her. Veronica came in and we’ve been seeing you for how many years now?

For this case, we really helped you in two manners. Number one was, we helped you with your lower teeth, and to make them look better. And number two was, we were able to position your jaw forward so you can go behind wearing the one at night time that you normally would wear. Then, this one, for function, so people don’t even know that you have anything like that going on.

Hi this is Dr. Antolak from thegentledentist.com at Shelby Township Michigan. 586-247-3500 is our phone number. I have this skull here because I’m showing what natural teeth look like, actually on dead person. I want to compare that and contrast it to what an all porcelain crown would look like to protect your teeth. And to redo the teeth. So what I want to first of all look at is natural teeth and what makes up a natural tooth. Just a little bit of details about that.

Natural teeth are made out of enamel. The outside layer is enamel, okay? And then the inside layer is called dentin. Dentin is a yellow color. And dentin actually gives you the ability to have pain. Dentin does not contain the nerve inside of it. But what happens is that small little tubules of almost like straws. Very, very microscopic straws inside of the dentin. And when you get cold, and you get cold, moderate sensitive or sometimes a toothache, things like that and it will if air is blowing across your teeth and it’s sensitive, these little tubules are full of water. They have water in them, a liquid and so what happens is, as this liquid moves back and forth within these tubules, it can promote pain within the pulp of the tooth. And, therefore, you can feel a sensation for pain. Okay?

Now, also with natural teeth, the reason why they look like they do is, they allow light to shine through the whole thing. As you can see on these. And you all know, that there’s no metal in natural teeth. Natural teeth, the enamel and the dentin, allows light to be transmitted through the tooth. So what we try to do is when we do our cosmetic dentistry. For example, if we try to use an all porcelain crown like this, we can see that it’s all white. Not only is it white, but it will actually transmit light through the crown itself, or through the veneer. So that it gives it a natural coloration and a natural appearance. In relationship to its, maybe a neighboring tooth that’s a natural tooth. So that it will actually allow that to be transmitted into appear very normal and natural. Sometimes, you’ll notice that if people have a dark line when they smile, on their tooth. Or their tooth looks really, really white and kinda dead. That’s because it actually has metal that covers over the tooth preperation, and when there’s metal, it does not allow for light to naturally become transmitted through it. But when light comes in it hits off the tooth and comes right back at your eye and that process of doing that can make it look natural and make it look opaque color because what the laboratories have to do when it’s metal underneath there, they have to put a very thin layer of white.Very opaque of white so that it makes it look natural. But the problem is when it tries to reflect light, it loses some of that natural appearances and it looks kind of dead. It looks too white.

Dr. Antolak: Hi, this is Dr. Antolak from thegentledentist.com in Shelby Township, MI, and sometimes people have issues with having emergencies where they do some trauma to their front teeth. And, our friend Aaron here, can you describe what happened?

Dental Patient: Well, I was trying to remove a socket from a ratchet, and slammed the ratchet right into my face.

Dr Antolak: And, what happened is right here. These teeth broke off here. So, right now, the most important thing was he saved a couple of fragments. This broke up in to a number of different pieces, but you can kinda see, this is a piece that we’re going to end up using. We’re going to actually bond this segment to his tooth here. And the other one’s going to get bonded to the other tooth. And then we’re going to basically back fill it with the composite materials that we use. Which are light cured. And we will see what that looks like as we do it. But we’re going to take our bonding agent, and prepare both sides of the teeth. And kind of glue it together, and then fill it with other materials. And so we’ll show you what it looks like. Okay.

So we’re going to go ahead and we’re just, that’s the surface of the tooth. Now we’re applying what is called a bonding agent right to it here. What this is going to do, is this is going to make it so that we can bond these segments together in pieces and so even I’ll apply this to the fragment of the tooth. He called yesterday. It was a Sunday and I said store it in water. He doesn’t have any pain so that was good. And so we are just going to apply a little bit of that to it and then we are going to blow up. So right now, those 2 pieces that I had… this is part of.

Here that has broken. We don’t need a light. And the other part is right here and we can see here, these are the 2 pieces. So we’re going to have to try to create some aesthetic appearance now between these segments that are broken so that it looks nice, and finish it up. So we just applied a thin material here and then bonded them together. And then we’re going to make sure they’re done. So, we’ll see what it looks like we’re done. Come in. You ready?

Dental Patient: Mm-hm.

Dr. Antolak: Okay. So we have a base color here that is going to be a little bit darker and we can notice on his teeth, that we have a little white opaque color to it. So we’re going to have to make this into a couple of different colors. This is kinda like taking a wallpaper for example, and part of that wallpaper got torn out. Now we have to, without having any other wallpaper samples now we have to try to make it look good. So as we can see in his teeth he’s got, this is like a base color underneath it we’re going to leave a little bit of a space there. And we’re going to put an opaque white color in there. And to kind of frost it and make it look like it blends in with his other teeth.

So this is called composite material, and, we can, reshape it and it works wonderful. This is actually what we do all our fillings out of, the front fillings or back fillings, and it allows us to give it the natural color of a tooth and yet have very nice aesthetics so it looks good. So we’re going to cure this now with a light. Then we have a special orange shield that we can do over that, so we’re going to just let that harden up. That light hardens it up.

There’s an opaque color to go over that to give it a little bit of different depths. So if we look at this it looks that same color there, but his teeth you can see he’s got where his natural tooth was here, he’s got this white frosty appearance to it, so we’re going to try to duplicate what that looks like a bit more. So we’re going to take a little bit of material here.

And, kind of, get it through the filling itself and, give it a little bit of a speckled look to it. Then we’re going to cure it with a clear material.

Dr. Antolak: So, we went ahead and finished this up we cured it. And did what we could to, kinda, match the colors and make it look more natural for him. And Aaron took a look, and that’s our final result right there, okay.

Aaron’s got his smile back now. We can be happy with that. So now we can put this on YouTube and everybody’s going to see it. You have any thoughts about that, about maybe taking something off of socket?

Dental Patient: Yeah, I’m gonna be careful about that now.

Dr. Antolak: So we’ll do a do that like I told you before is that now that tooth has been kind of traumatized a little bit so we’re going to have to see what happens over time. In a situation like this it’s been hit and jarred pretty hard. So we’re going to see how the nerve responds, if the nerve responds normally. Or it might need to have the nerve removed in the future if it doesn’t. So we’ll keep an eye on that.

One of the most commonly asked questions in our office is about cosmetic dentistry. In other words what are my options to improve and keep my smile? There are actually several things which can be done to improve the smile. Each one is successful and are limited to what they can do.

Teeth Whitening

Whitening or bleaching your teeth involves using a “supercharged” peroxide which cleans out the pores of the teeth allowing them to be the brightest possible. While there are several techniques available the most common one involves using an in office procedure to make custom stints (mouthguard) of your teeth. The active gel is placed in the custom guard and worn at home for a period of two to three weeks. Usually after a year or so a touch-up procedure can be done to keep them brilliantly white. The difference between those made at the dental office and the “store or TV” bought products is in the professional supervision of a dental professional. The trays are uncomfortable in the generic products and the material is less effective. Cavities should be taken care of before the whitening process is started and the whitening agent won’t change the color of fillings or crowns.

Plastic Resin Fillings

Resin fillings are available which are almost a perfect match for the color of your teeth. Under most circumstances they can be used in place of the old silver fillings. They are placed like putty in your teeth and “cured” by a blue light which makes them hard. The old type of plastics turned yellow over time making people look like they had stuff stuck between their teeth but the newer plastic resins are more color stable over time. These fillings used in the back teeth have limitations in the size of the filling they are replacing.

Porcelain Onlay and Crowns

If the filling is large the tooth can have a full porcelain onlay or crown. These are beautiful and look like a natural tooth. They are stronger than regular black or resin fillings and can have a better appearance than porcelain fused to metal crowns. These are bonded to the tooth and because they are made in the laboratory they are accurate and very durable.

Bonding

Bonding involves painting a putty-like plastic over the teeth to cover up the old color and sometimes even change the shape of the teeth. This plastic resin is applied to the teeth in an artistic way to form and enhance a person’s smile. This plastic will eventually wear and will need to be replaced or added to over the years. Most of the time you can expect 3-5 years of wear depending upon your particular situation.

Veneers

Veneers are a porcelain covering that almost fuses to the teeth. These are the most durable and beautiful of all the cosmetic procedures. The porcelain allows for natural light to penetrate through the tooth and veneer giving it a very natural appearance. The porcelain veneer doesn’t darken over the years from aging or staining like natural teeth and resin fillings do.

They want to show bright white straight teeth. And what happens when people don’t have bright white straight teeth? Well, they want to get them straightened out and whitened.  Well we can do the whitening through a number of different options. But one of the ways that we’ve found that works really well in our office, in my hands, is something called ClearCorrect. ClearCorrect is an aligner, just like Invisalign is an aligner and this aligner in ClearCorrect is great because what it does is it gives us a little bit more versatility than the Invisalign product does, it also is a bit more reasonably priced and so we can pass that savings on to our patients. In fact I am using one right now as I’m speaking and it looks like this.

What it is- it goes in the, in the mouth, in the teeth and it moves the teeth, it’s made by care designs. So it’s made where you can actually see each step of the way, it actually moves the teeth so you put a new one every 3 weeks and when you put it in you feel a lot of tightness for a day or so and then it slowly goes away and the teeth moves a little bit and every 3 weeks you put in a new one. Can you put this back and.

So it’s that easy to put it back in, people don’t notice. So the way that, the differences between Clear Correct and Invasalign is that with Clear Correct the brace itself, the aligner is actually clearer than other aligners. This is according to their literature they have. But in my hands, it also handles better too.  Also it does able to control what happens with the teeth. For Clear Correct they actually send, which is pretty neat they send their little templates that are made. They make a model and so then if you for example, lose them you can come in, we can make another alignment for you right here as you lose, if you lost it then you can do it. And, there is no additional charge for that up to a couple of aligners if you do misplace those or lose them.

The other thing is that, that since we do it remove the teeth actually a little bit slower, so is actually the end result is predictable, and the nice thing is because it’s shaped like a lightning tray, we can use those also at the final end product to, to whiten your teeth up. So there are many different advantages to Clear Correct. And it’s something that we use in our office and we use it mighty effectively to move teeth without braces, just like the Invisalign does. And it’s a more beneficial product. They’re a little bit easier for us to work with as dentists than the Invisalign product. Clear Correct at The Gentle Dentist in Shelby Township, Michigan

Today I want to talk to you about dental crowns and specifically cosmetic all porcelain dental crowns verses porcelain fused metal crowns.  When you look at somebody in the front and they’re smiling and you see a gray line around the edge of the gums, it’s not so sight, it’s kind of unsightly and the reason why that is because porcelain crowns allow light to show through and what you’re looking at, the grayness at the gum line or, a dark line is a porcelain infused development.

Now, porcelain fused to metal crowns were the standard years ago. But nowadays, with no new current technologies, we’re mostly doing actually all porcelain crowns. Not only for the cosmetic benefits of having a beautiful smile with, with cosmetic with all porcelain. But also because they now are stronger than they ever have been. What I’d like to do is just demonstrate to you a couple things. I have here a light and then that light is showing.  If you can see that, it’s showing through a porcelain fused to gold crown. This is a porcelain fused to gold crown and there is actually porcelain that’s baked on the outside on the outside of a tooth and then there is gold that’s ore metal that’s on the inside and that does give it the strength it needs. Now, the problem is, is that they actually and the actual teeth, and the actual teeth allow light to show straight through them. So for example, on this all porcelain one that I have you and see that there is light that is allowed to emit right through that and when light is allowed to be emitted right through the tooth, what it does is give it a natural appearance, just like a natural tooth. A natural tooth there is no metal in it of course, and it allows light to penetrate through it, just like I demonstrated to you with the all porcelain ones. The reason why it looks dark underneath it is it may have a band of metal that’s at the gum line but also

There’s no light allowed to be emitted to the root of the tooth. And so you’re basically looking at a shadow that’s coming into the root structure of the tooth.

Now many, many times I have patients who have come in and asked me, why is my gum line looks so dark there at the, at the top and so when I smile, I don’t like when I smile because I see how that looks and, and the tooth looks a little unnatural, because the naturalness of it comes from the fact that it’s level and we have to, they have to put an opaquer on there like a real whitener that blacks out the metal look from the porcelain. And then they have to make it look natural. So rather than lighting emitting through the tooth on a natural tooth or an all porcelain crown.

Like it’s trapped inside and when you’re looking at a porcelain crown.  Light only goes up to that one specific spot where the crown is, where the opaquer is, and light reflects back off of it. That’s what not, does, that’s what gives it the unnatural look to a porcelain, I mean a porcelain to metal crown.
