At The Gentle Dentist located in Shelby Township, Michigan we perform both root canals and place and restore dental implants so I would like to say that we are completely objective when it comes to decision making.

Root Canals are used to save teeth when the nerve tissue becomes infected. When cavities develop so deep into the nerve the bacteria from the mouth  penetrates into it and creates pain and eventual infection.  This destruction of the nerve tissue creates throbbing pain, pain from hot or cold or pain to biting down.  The infected nerve tissue may develop an abscess which can cause swelling around the tooth.

Root canals are usually the first line of treatment with teeth to save them.  Extracting teeth that are infected is also an option to eliminate pain, but the problem is how to replace the missing space.  This is where implants come into play.

Implants are titanium cylinders that are inserted into the jaw bone.  They can have crowns placed on them to restore the missing space where a tooth was previously placed.  Implants are used if a root canal fails and it is not practical to retreat the root canal.

Root canals and dental implants are both used to restore or replace the missing spaces between teeth. They both stimulate the bone to prevent the bone from shrinking.

at www.TheGentleDentist.com in Shelby Township Michigan we will typically replace broken down root canal teeth with a single tooth implant. In this case you will see an x ray where the front tooth didn’t have a post and it literally sheared off. Without any tooth above the gum line it was not possible to redo the crown.
The benefit if replacing a tooth with an implant is that an implant will be stronger than the brittle root from a root canal tooth. It is necessary to have enough natural tooth above the gum line to properly restore it so that it lasts a long time.
In this video you will see some blood as we show the implant going in and you will see the root of the tooth. A major benefit of an immediate implant is that the implant is placed at the same time as the root is removed so that he didn’t have to go without a tooth in the front.

I have patients come into our Shelby Township Dental Office asking advice on whether to have mini implants or standard sized implants placed.  This is a fairly common question I get and want to give some information which describes their commonalities and their differences.   Generally, I would have to answer  that the basic differences in them is the flexibility and what they are going to be used for.

Dental implants are made of titanium, a metal that is completely compatible with the body.  The human  body does not have a reaction to this metal.  Very few metals are bio-compatible which means that they do not create an immune response in the body.  This is what has made implants successful over the past 40 years or so.

  • The standard dental implant is usually made of 2 pieces and is greater than 3 mm in diameter   The standard dental implant has an external screw that allows it to be screwed into the bone and an internal thread which allows different components to be torqued down into the dental implant.    The inside threads of the standard sized implant allows different  components to be attached to them.

The components or abutments are used to:

  • replace missing teeth or a single tooth.
  • Hold  removable dentures in place so they can be snapped on and off. These abutments are like “snaps” and because they are screwed into the dental implant they can have a stronger hold than the mini dental implants

There are many different manufactures of these standard  dental implants that have their own shape and design.  The good news is that they all work very well with a over a 95% chance of success.

  • Mini dental implants are a solid one piece screw that is less than 3 mm in diameter with a specialized ball shaped end that sticks out of the jaw bone.  Inside of the denture is a rubber O ring that fits around this ball.  When the denture is placed over the ball the rubber O ring keeps the denture retained.

Comparisons:

  • Because the mini dental implant is only 3 mm in diameter it needs to be 1 piece for strength. The 2 piece standard dental implants have enough diameter so that an internal thread can be placed inside of the top of the dental implant and keep it strong enough.  When comparing the two systems it is necessary to use four mini dental implants to hold a lower denture in place whereas two standard implants would be comparable.
  • The cost of a standard implant is about 50% more because of the fact that there are more components and because the surgery is a little more advanced.
  • Mini dental implants are good for those who are in poor health because no flap of gum tissue needs to be made for these.  Because no flap is needed it also becomes more challenging because the bone in these patients is usually very thin and it is hard to determine where the ridge of bone is.
  • Because the standard sized  dental implant has individual components screwed into them the become more universal and can be used for different purposes.

I can do and have done both mini dental implants and standard dental implants in my Macomb County Dental office but I prefer the placement of standard dental implants if there is enough bone because it is more predictable to use in my hands.

For over 100 years, dentists have been using silver-mercury amalgam fillings to restore decayed or broken teeth.   These materials contain about 50% mercury combined with silver and other metals.  The worked well but they are not only ugly and unnatural in appearance, but they have the potential of creating fractures in the teeth cause by the expansion and contraction of the mercury within the filling.  In the past the only other alternative was gold and that was too expensive for most folks.

For about the last 15 years or so there has been an economical way to beautifully restore teeth in the Shelby Township, Sterling Heights area  using tooth-colored resin fillings.  The resin is made up of glass particles in a plastic matrix which is like clay as we mold it and shape its surface.  We bond the putty-like material to the tooth with a high-intensity light.  This hardens the material and allows it to be shaped and polished for immediate use.

Once completed, the tooth has a natural appearance and is actually stronger than before due to the strength of the adhesive agent used to bond the resin to the tooth. Because these fillings bond to the teeth they can remain very small whereas in the past the silver-mercury fillings had to have a large hole drilled to hold the filling in place.  Resin restorations not only look better, but they are stronger and last longer than the old silver-amalgam fillings.

Resin restorations are slightly more costly than the amalgam fillings because of the time and expertise required to place them is much greater.  The benefits of increased natural beauty and safety are worth the difference in price.  Insurance usually covers the amount that the silver amalgam filling would cost and you, the patient, are responsible for the difference.  The price range is from $150 to $350 per tooth.

The use of Cosmetic porcelain veneers is a very common way that many Michigan residents choose to enhance their smiles.  Porcelain veneers are thin shells of beautiful porcelain that is fabricated in a dental laboratory.  They are about the thickness of a credit card and are very fragile when handled.  When they are bonded to the natural tooth they become incredibly strong and are about as strong as your natural teeth.  Every attempt is to keep the shaping of the teeth as conservative as possible.   Lumineers are a name brand for a type of porcelain and placing them on teeth gives a natural look.

The following is an example what laminate veneers are used for:

  • Straightening crooked teeth by filling in the areas that are pushed in and in areas where the teeth are protruding slightly more tooth reduction is necessary.  The idea is to get the teeth to look uniform when looking at someone straight on or to the side.
  • Darkened teeth can be made whiter.  This is done by reducing some of the thickness of the natural tooth and thickening it up with whiter porcelain.  The same principle applies when Formica is placed over a kitchen top.  Any malformations are covered up by this thin film so no one will know the difference.
  • Missing teeth can be filled in with a fixed bridge made of the same porcelain as the veneer.  This is a very natural and conservative approach to a beautiful smile when a tooth is missing.
  • To close spaces between teeth.  This is very common and by making the tooth slightly wider the spaces can be closed up, giving a beautiful result.

Dentures: How to prevent getting dentures, taking care of them and their current trends.

There are so many topics in dentistry that I can write about in this column since research is exposing more advanced techniques, materials and objectives in the field.  This month I wanted the Shelby Township, Macomb, Sterling Heights and Clinton Township readership to become aware of the state of edentulous (without teeth) patient and the struggles they go through.  It is also important to know that the percentage of  people who have lost their teeth is increasing even though research is advancing.  The problem with tooth loss is that the skeletal bone actually shrinks over time where the teeth have been extracted.  Since dentures rest on the gums people who have dentures only can bite down 15-20% as hard as they could when they had teeth.  This means that they have a limited assortment of soft foods they can comfortably eat which tends to not be as nutritious as harder foods such as fresh fruits and vegetables. 

Demographics:

According to an Academy of General Dentistry article there are around 35 million Americans who have either 1 or 2 jaws of missing teeth and this number is expected to increase to 38 million by 2020.  26% of the US population between the ages of 65-74 do not have teeth.  According to David Felton DDS, the demographic most at risk for dentures are lower education, smokers, females and a population who does not take daily vitamins or supplements. 

Tooth Loss

Once thought that dentures were a normal age related event we now know it has a lot to do with genetics, nutrition, and oral hygiene.  Those who are prone to tooth loss at an early age from cavities and periodontal disease are at a much higher risk for complete tooth loss and eventual dentures.  Once a tooth is removed, the adjacent teeth move into the space creating gaps where food gets lodged, creating more problems and more tooth loss.  This is why it is best to save teeth with root canal treatment if a tooth is infected rather than have it extracted.  Having fillings done at an early enough stage prevents root canals or extractions.

 

Periodontal disease is the leading cause of tooth loss in adults.  This bacterial induced disease is not painful and has little symptoms so it must be diagnosed and treated by a dentist before it is too late.  If your gums bleed when brushing or flossing, if you have bad breath, if your teeth are loose, if there is swelling around the gums or you have tarter buildup then you more than likely have periodontal disease.  Ignoring your problem won’t make it go away, in fact the sooner you are treated the better the outcome. 

Denture Care

For those who have had their teeth already removed and have dentures there is hope for you.  First of all, dentures have to be replaced or relined every 5-8 years.  Because the bone that supports the denture continually shrinks it is very important that you maintain the best fit possible.  When dentures are too loose they irritate the soft tissues and bone underlying the denture.  With less biting pressure the diet only permits for softer foods to be eaten.  Another factor not addressed much is the potential for an embarrassment moment when laughing with a denture and having it drop or even pop out of your mouth. With a good fit dentures provide confidence to be yourself and to smile and laugh without concern.

 

Assuming the dentures are made properly and fit relatively well, mini implants can be placed in one visit to provide a snap fit so the dentures are stabilized.  These implant supported dentures continue to rest on the ridge, but since they hold tightly to the gums without adhesive, they don’t move.  You can bite, chew and laugh without worrying that the denture will jump or fall out of place. 

 

Here are some pointers on how to take care of your dentures:

  1. Don’t use tooth paste to clean them-This can scratch the surface and potentially promote staining and tarter build up.
  2. Make sure you remove them when sleeping.  When removing the dentures at night the soft tissues to get access to body’s healing saliva of the mouth.  When leaving in place thrush can develop.  This reddish yeast infection can also be seen on the corners of the mouth. 
  3. Soak your denture at night in Efferdent, Polident or other soaks.
  4. See your dentist every year for a denture fit analysis.  The exam we do also provides for a cancer screening in addition to an analysis on your denture’s fit.
  5. Clean your dentures with a brush and a denture cleaning paste such as Denture Cream every day. 
  6. Use a wash cloth to rub and stimulate the roof of the mouth, tongue and lower ridge.  This prevents the build up of bacteria and yeast in the mouth.  It is also prudent to rinse the mouth with a mouthwash such as Listerine or Crest Pro Health.
  7.
