It’s a free newspaper for seniors that want to do things that are relevant for their age group. And I’ve been a columnist for about going on 11 years. And I always ask our senior patients what kind of questions they may have so that I can then create some questions and answers that are relevant. And the most recent questions I did have, and I’ve had it throughout the years is what is the cost of dentistry? Why is dentistry so expensive?

I want to just address that issue in a couple of ways. First of all, I’d like to talk about a little bit about the facet of the expense of providing quality dental care and also from the dentist’s perspective.

First of all, the cost of dental care is much more expensive than one would imagine just kind of walking into a dental office. And they think that there are no additional expenses beyond that. But the reality is it’s furthest from the truth. You know, to provide high quality care, we need really good people who work for us. And to do that they cost money. And every person who works at the dental office is compensated quite well. And now in addition to that, there’s the cost of doing health insurance; you know medical insurance with that and with all the other aspects that come along with employment issues. And those are usually one of our highest expense are the cost of employees.

Now we have also the condition that we have the cost of the facility that we provide our care in. It’s got to be a clean place. It’s got to be kept up to date, so that we are clean and it reflects our values and who we are at our office at The Gentle Dentist in Shelby Township, Michigan.  Other things that are really important are to keeping up on high technology.  High technology in everything has touched every part of our lives. And because it has touched every part of our lives, and dentistry is no different. You know, we have the technology such as our computers – we have to make things more convenient for you, our patients.

We have a paperless office which requires computers so that it makes it more efficient for you and there’s actually less waste in the environment that way. We have the cost of digital panoramic X-rays. What that does, it saves time and it saves radiation, so you’re not being exposed to that much. That all costs money and how we develop the cost of providing care. We have to provide high quality care for laboratories that we use for us at the office, we use somebody that’s local; actually someone who lives in Macomb Township and he does wonderful work. So, all the work and all the treatment are done with American made products by American.

Unfortunately, sometimes you find out at some of these low-cost clinics, is a lot of times they’ll send all their lab work out to China and then you never know what kind of inferior products you’re going to receive from that, too. In addition to that, the second part of that answer was the dentists themselves. You know as a dentist, I’ve been practicing for 26 years but those dentists who come out of dental school now, are at least 100 to 200,000 dollars in debt before they even begin and so they’re starting way behind the 8 ball. And not only that, is as dentists, number 1 is we go to undergraduate school and we go to graduate school. The combination of those 2 is at least 8 years of school. And so that’s 8 years of being out of the potential earnings that we would make up if we were, if we were employed.

So it may seem like a lot of money and these types of things- it is expensive. I’m not denying that for one moment. But the cost of care, the cost of keeping up with the government regulations, with Obamacare, these things are all impacting us as dental professionals. And by the way, would you want to go to somebody who’s cheap? Because usually what I find is patients who come to see us are those who have been at some of these cheap clinics who provide free care. And the reality is, its inferior products and many, many times it doesn’t work for them and they have continued failures with the process. So, you know you have to ask yourself the question, is it worth your time. Is it worth your money to pay a little bit more to get high quality care that is guaranteed, which we do in our office. And then you know that when you get something, you’re going to know that it’s done right and done well.

Hi, this is Dr. Antolak from in Shelby Township Michigan, (586) 247-3500 is our phone number. What things do people mostly want to see when they have their smile? They want to show bright white straight teeth. And what happens when people don’t have bright white straight teeth? Well, they want to get them straightened out and whitened.  Well we can do the whitening through a number of different options. But one of the ways that we’ve found that works really well in our office, in my hands, is something called ClearCorrect. ClearCorrect is an aligner, just like Invisalign is an aligner and this aligner in ClearCorrect is great because what it does is it gives us a little bit more versatility than the Invisalign product does, it also is a bit more reasonably priced and so we can pass that savings on to our patients. In fact I am using one right now as I’m speaking and it looks like this.

What it is- it goes in the, in the mouth, in the teeth and it moves the teeth, it’s made by care designs. So it’s made where you can actually see each step of the way, it actually moves the teeth so you put a new one every 3 weeks and when you put it in you feel a lot of tightness for a day or so and then it slowly goes away and the teeth moves a little bit and every 3 weeks you put in a new one. Can you put this back and.

So it’s that easy to put it back in, people don’t notice. So the way that, the differences between Clear Correct and Invasalign is that with Clear Correct the brace itself, the aligner is actually clearer than other aligners. This is according to their literature they have. But in my hands, it also handles better too.  Also it does able to control what happens with the teeth. For Clear Correct they actually send, which is pretty neat they send their little templates that are made. They make a model and so then if you for example, lose them you can come in, we can make another alignment for you right here as you lose, if you lost it then you can do it. And, there is no additional charge for that up to a couple of aligners if you do misplace those or lose them.

The other thing is that, that since we do it remove the teeth actually a little bit slower, so is actually the end result is predictable, and the nice thing is because it’s shaped like a lightning tray, we can use those also at the final end product to, to whiten your teeth up. So there are many different advantages to Clear Correct. And it’s something that we use in our office and we use it mighty effectively to move teeth without braces, just like the Invisalign does. And it’s a more beneficial product. They’re a little bit easier for us to work with as dentists than the Invisalign product. Clear Correct at The Gentle Dentist in Shelby Township, Michigan

Today I want to talk to you about dental crowns and specifically cosmetic all porcelain dental crowns verses porcelain fused metal crowns.  When you look at somebody in the front and they’re smiling and you see a gray line around the edge of the gums, it’s not so sight, it’s kind of unsightly and the reason why that is because porcelain crowns allow light to show through and what you’re looking at, the grayness at the gum line or, a dark line is a porcelain infused development.

Now, porcelain fused to metal crowns were the standard years ago. But nowadays, with no new current technologies, we’re mostly doing actually all porcelain crowns. Not only for the cosmetic benefits of having a beautiful smile with, with cosmetic with all porcelain. But also because they now are stronger than they ever have been. What I’d like to do is just demonstrate to you a couple things. I have here a light and then that light is showing.  If you can see that, it’s showing through a porcelain fused to gold crown. This is a porcelain fused to gold crown and there is actually porcelain that’s baked on the outside on the outside of a tooth and then there is gold that’s ore metal that’s on the inside and that does give it the strength it needs. Now, the problem is, is that they actually and the actual teeth, and the actual teeth allow light to show straight through them. So for example, on this all porcelain one that I have you and see that there is light that is allowed to emit right through that and when light is allowed to be emitted right through the tooth, what it does is give it a natural appearance, just like a natural tooth. A natural tooth there is no metal in it of course, and it allows light to penetrate through it, just like I demonstrated to you with the all porcelain ones. The reason why it looks dark underneath it is it may have a band of metal that’s at the gum line but also

There’s no light allowed to be emitted to the root of the tooth. And so you’re basically looking at a shadow that’s coming into the root structure of the tooth.

Now many, many times I have patients who have come in and asked me, why is my gum line looks so dark there at the, at the top and so when I smile, I don’t like when I smile because I see how that looks and, and the tooth looks a little unnatural, because the naturalness of it comes from the fact that it’s level and we have to, they have to put an opaquer on there like a real whitener that blacks out the metal look from the porcelain. And then they have to make it look natural. So rather than lighting emitting through the tooth on a natural tooth or an all porcelain crown.

Like it’s trapped inside and when you’re looking at a porcelain crown.  Light only goes up to that one specific spot where the crown is, where the opaquer is, and light reflects back off of it. That’s what not, does, that’s what gives it the unnatural look to a porcelain, I mean a porcelain to metal crown.

Listening to our patients needs that type of thing. We also want to, the amenities that we try to provide are sooner the other things too. One of the ways we do it is through really neat technique and that’s using the device that I am holding up here, actually have these for sale, and this is called hideout.

And what it does is it is an Atenge unit, which helps to create electrical stimulation in muscles, in different muscle groups, that creates relaxation and it creates release of tension as if it were being done physically. It’s done thru electronics and thru little electrical impulses. And let me show you how that works. This unit is small which is really nice thing. A lot of times people had Atenge units that are large, but this very small. And what it does is if you can actually plug into your computer charge up or into a wall, to charge up and has 6 different setting each setting one might feel like um like it’s a ball rolling around and some is might feel like it’s squeezing your hands and we have these leads that go on your, your bare skin and these leads are the things that provide the electrical stimulation and this unit has the ability to do two separate leads. So you can be massaging your back and massaging your lower back and massaging your neck at the same time which is really nice thing.

For me personally earlier this summer I was using this on a trip to where I was in Washington, DC with my family and there was a lot of walking around and my lower back was giving me some issues.  So I just put this on, on the lower part of my back as I’m walking around the museums and I’m literally getting charged through the whole time I’m going through these museums and I just had this in my pocket.

It lasted all day if you can believe it and it provided really relief for me during that time, included with this set up that we have is, this is the battery charger that it comes with it and plugs right in the very end. It plugs in to these USB cable and it gets, it has a couple of different leads and then some, some different wires that go with it too. But the beauty is, this device here, and this has the acupuncture pressure points through the, through Chinese pressure points. And so each area here is like a meridian.

And in these meridians, these are little points that you can apply for different purposes. On the backside of this manual it has in English, it actually has where, what area if you’re having discomfort in a certain area and you go back and you reference back onto here, and if you can reference back on to here it provides really good relaxation for different parts of muscle groups. One thing that we do here at our office is if someone has some temperamented joint pain or problems in the jaw joint. What we’ll do is we’ll actually apply these leads to the, the hands, like about, right here.

And where it’s applied, like right here, it creates, and don’t ask me how it works, but it’s, you know, through thousands of years the Chinese have perfected this. It’ll create relaxation in the whole muscles and the tightness of the face. There’s also some areas where you can place it on these muscle groups here, on the temporalis area  but you, you have to be, you know, use it on a very low setting for that purpose. It’s very safe. It’s very effective, and the nice part is that this device is rechargeable and it’s very portable. Hi, this is Dr. Antolak from The Gentle Dentist in Shelby Township, Michigan. What I’d like to do is speak to you about emergency dental care. At our office, we try to make ourselves available to those people who are having dental emergencies. And normally, dental emergencies happen at the most inopportune time. So if you’re trying to reach us during off hours we’ll definitely get back with you as soon as we can.

Dental emergencies are basically two or three different types of emergencies. One is traumatic, and that is if you get into accidents of some sort where you can break off your tooth and then you see either have that repaired or if you have an emergency with tooth actually falls out. Rinse that out and place it right back into that socket that where it came from. That’s the best of taking care of your problem with that. The other type of dental emergencies that we do have are when someone has had a filling missing or a big hole in the mouth. And all of a sudden, the thing develops into a bad toothache. Well that toothache is from the bacteria that’s in the mouth getting to the nerve. And then it goes from the nerve into the bone. And as it gets into the bone, it creates an infection, and that infection creates pressure inside of the jawbone. And that’s a dental emergency that gets that taken care of.

And the third type’s is where you normally see is where somebody has gum disease and they have bone loss around their teeth. Well they might get some food caught in that area or even around wisdom teeth, where food gets caught in that area, and blows the wisdom teeth up. It hurts and then they have to have something to deal with the pain. There’s a few different ways of dealing with emergencies that do take place over the counter remedies are obviously pretty limited.  But the most important thing is to get the dentist as quickly as possible.

The Gentle Dentist Discusses Emergency Dental Care in Shelby Township, Michigan

Hi, my name is Jessica, and I’m a hygienist, and also the patient coordinator here at Dr. Antolak’s office, the Gentle Dentist in Shelby Township, Michigan. I’m here today to talk about a common dental problem, which is sensitive teeth. There are many reasons why people have sensitive teeth. It could be because of decay or many other reasons. And until you come in for an exam with Dr. Antolak, we will not know the exact reason. I’m here today to talk about dentin hypersensitivity. You might have seen it on a commercial on TV. But if not, I’m here to explain what it is.

It is a tooth problem that can affect over 40 million adults in the U.S. It usually affects people ages 20 to 49. But it can also affect teenagers and also older adults.  It’s caused by loss of enamel that shows the underlying layer in the tooth called dentin. And, here I have the diagram. This is what a tooth looks like on the inside. A cross-section of a tooth.  Here this white part that you can see when you see your teeth right here, is the enamel. Which you’re pretty familiar with, and also, it’s one of the strongest substances in the body. Underneath the enamel is the dentin, this yellow color, orange color right here. It is a very softer substance than the enamel. Underneath the dentine is where the nerves and blood vessels lie.

So, dentin hypersensitivity occurs if you have receding gums or a loss of enamel. And, the reason you experience a sensitivity is because, in the dentin, are tubules and so, if something that’s hot or cold, acidic, any other stimulus, hits the dentin, it’s more than likely to make an impulse that’s going to excite the nerves in the tooth. And you’ll get sensitive or painful teeth.

So what can you do if you do have this problem? We have many options here at Dr. Antolak’s office. One thing we might recommend for you is a tooth paste that contains potassium nitrate which is found in sensodyne or other sensitive toothpaste.

What potassium nitrate does, is it blocks the nerve impulse. So that will decrease sensitivity that you might experience. We also have a new product here at our office called MI Paste. It’s a milk based product that contains calcium. So if you’re allergic to milk, you cannot use this. It reminds me, actually of a yogurt. And it helps to remineralize your teeth.

Hi, this is Dr. Antolak and Jessica from, we’re in Shelby Township, Michigan. And our phone number is (586) 247-3500. You’re probably wondering what I’m doing with this. This is called a pulse oximeter. We use this device when we provide oral sedation to our patients. What it does is it has the ability to detect the blood pressure, the oxygen saturation, the oxygen that’s in your blood and your, your heart rate. So what I’ll have Jessica do is first of all put on the blood pressure cuff and this is all automated and so this goes on like that and then also we have what’s called a pulse oximeter and through this device, there’s a little red light there and what that does is that red light that’s in there measures through your skin and through your nail bed.

The amount of oxygen, don’t ask me how it does it, it just does it. Through the oxygen through there. So, Justin you could go ahead and push the start button.  And what it’s going to do is it’s going to inflate the blood pressure cuff. And it does it automatically, which is very nice. And that way, we can see our patients, and treat them throughout the whole process, then we can monitor the blood pressure. We actually have probably around 6 or 8 different patients. We’ve been able to screen them and find out that they had high blood pressure that they didn’t know of. I mean serious high blood pressure. So this is 1 of the parts that we do during our initial visits to provide a screening exam for that.

What way if somebody does have high blood pressure you can send them to a physician and then they’ll know.  If they do, then we try to do some things to help calm them down. At the same time, we want to make sure that their pulse rate doesn’t slow down too much, or their blood pressure doesn’t go down too low. So we just have a good estimate.  The good thing is when they do oral sedation, it’s very safe and very effective when people, when they leave and I call them later on that evening many times they don’t even have any clue or memory of, of the procedure that was done with them. This is once again is a very.

One of the ways that we try to provide the most safety that we can for our patients, and a way that keeps us, just more in touch with modern technology. This is Dr. Antolak from in Shelby Township, Michigan. Every time I do a video I always have these on. And I’m certain that those of you who do not know what these are, you may be questioning that. These are called dental loupes, and what they do is they allow us to magnify wh0at we see for me at least for these size 4 times magnification so something that might appear as if it’s this big literally in the mouth, let’s say a tooth is maybe this big in the mouth for me it appears as if it’s this big through my loops and so that gives me the ability to see things in more detail, to fight better dental excellence and in my examinations to provide dental better dental excellence in the treatment that I’m providing.

You know, it also was really good for my back because it helps to keep me up straight because it doesn’t allow me to get too close to the patient. It also helps us to be more accurate in our assessment of what our patient’s, what they see. Also in addition to that we have a light, this is a halogen, not a halogen light this is a LED light that can see right there and what that does, by using that it allows us to see a very specific spot where I focus my lighting right on the patient and in the mouth and so I can illuminate a lot more things and once again pay more attention to detail to provide better dental care and better dental excellence.

Hi, I’m going to talk to you about our comprehensive exam today. Our comprehensive exam is a little more extensive than most exams that you’ve probably had in the past. It starts out by you coming to our office and we’ll do a full exam of the head, neck, and oral cavity. The process will start with us receiving your medical and dental history through some forms that you’ll fill out online.

You can either do this from home or at our office, when we take you back we will go over those forms with you, talk about any concerns that you may have, then we will begin with a full series of x-rays and photographs to help us diagnose any decay or anything that might be going on. The Gentle Dentist in Shelby Township Talks About the Comprehensive Exam

Well, this is a dental instrument, as we all know what it is and a dental syringe.  We basically can put anesthetic, which is a liquid material, inside of here. And of course, there’s a small little needle that goes into the soft tissue. I know this makes people like really freak out, looking in I understand, but the reality is this is what we need to do to to numb you up.

Since we always strive in our office for the best and most excellent type of treatment and care, I’d just like to describe a few changes that we’re doing at our office.  1 it is not only do we use a topical anesthetic and leave it on the gum tissue for a couple of minutes, which totally numbs the gums up so you don’t feel any pain at all with.

The small needle that we showed you. But, also we have this device. What this is, it is called a Vibraject and what it does is it’s a little device that we have it clipped, actually clips right on the anesthetic needle itself. And what it does is, is it vibrates.  That’s kind of odd isn’t it?  It vibrates. What does the vibration do? The vibration will be distracting because it’s vibrating so quickly that it’s called the Gateway Theory to Pain.

Then it’s vibrating so much that when it’s doing that you don’t feel the anesthetic that’s going in because your body is feeling the vibration. And so this is just, do it in the same way we inject slowly so that there will be no blood but I plan to try this on a patient they loved it because they don’t feel a thing when we do it and so this is just a battery operated device that we can use to make your experiences here more comfortable and really your, your anesthetic that you know.

Hi, my name is Margie, I am a dental hygienist here for Dr. Robert Antolak, the Gentle Dentist in Shelby Township, Michigan and I briefly wanted to talk to you today about periodontal disease. Most people have no idea how a problem it is, the percentage is about 70 to 80 percent of all Americans have some form of it. It could be something as simple as just going to the dentist and having yourself checked each and every time you come in to have a simple screening done which consists of measuring the depth of how far underneath the gum you can go around every individual tooth. It’s something very simple, relatively painless and this can be done actually every 6 months you come in and if you do that you can prevent a major difference in the protocol that would have to be done if you had something that was more extreme, if you let it go for years. It could be something as simple just as a routine scaling, something a little bit more.

Step 2 would be a root planning scale, which is a more in depth scaling that we numb you up, so I can get all the way down to the pocket, remove all the debris, and when you heal, hopefully, those pockets will tighten up to the, closer to the tooth and have better results that way. Or, you’ve got something that’s more extreme and has to have, you have to have surgery. So, to prevent that, all you have to do is come to the dentist regularly, and it can be, noted on your chart and you, you have the charting done. We mark it, we check it each and every time. Just, again, to prevent anything from getting any worse.
